The 2021 Community Health Assessment (CHA) is a comprehensive and actionable picture of health and wellbeing in Wyandot County. Please submit any questions or feedback to wchealth@co.wyandot.oh.us.
The community also is invited to participate in the 2021 Community Health Improvement Plan (CHIP) process. The CHIP is Wyandot County's roadmap to address the many challenges identified in the 2021 CHA. The CHIP calls for cross-sector partnerships and alignment to meet a manageable set of measurable goals.
